When I shouldn't have been having any ideas
A D&D game where it's all a metaphor - The World is a human body and the PCs are immune cells
It looks like a FRPG world to the players and the characters, but the map, people, places are based on a human body - and the objective is to defeat a terrible disease that is infecting the host.
They must fight monsters based on diseases and conditions until they level up
BBEG might be something like cancer, twisted, horrible humanoids who divide in two and continue to grow at a geometric pace.
But first they have to visit the city of scholars the canyon of the winds, the great engine and so on, and gather lore and power until they're epic level and ready to fight
Hints that their world is a person, - seasons in the game represent the person's day, as they unknowingly have a meeting with family and a feast before their disease riots out of control.

It sounds interesting on the face of it, but might be a touch too Jar of Tang. What is the point to covering it with an FRPG world? Or from the other direction why is the body overly necessary?
